Request for Social Security Number Correction

 

On behalf of Saddleback and Irvine Valley College students, the South Orange County Community College District (SOCCCD), is required to report to the IRS all registration fees incurred and scholarships received.  If you received a notice that we do not have your Taxpayer Identification Number (TIN) or Social Security Number (SSN) on file, or the one you provided does not match your name, you are requested to do the following:

Step 1.  Complete Part I of the W-9S form to provide your correct TIN and name.

Step 2.  Complete the required Student/School information below.

Step 3.  Return the information from Step 1 and 2 using one of the methods below.

   Students may also carry the forms to their college Admissions and Records Office

What is Form W-9S?

Form W-9S, Request for Student's Taxpayer Identification Number and Certification, is used by eligible educational institutions to request the taxpayer identification number of students for whom the institution must report 1098-T Tuition Statement information to the Internal Revenue Service. We cannot report your Tuition information to the IRS without your SSN/ITIN.

Privacy Act Notice

Section 6109 of the Internal Revenue Code requires you to give your correct SSN or ITIN to persons who must file information returns with the IRS to report certain information. If you fail to furnish this information prior to payment, you can be subject to a $50 penalty imposed by the IRS.  The IRS uses the numbers for identification purposes and to help verify the accuracy of your tax return. The IRS may also provide this information to the Department of Justice for civil and criminal litigation and to cities, states, the District of Columbia, and U.S. possessions to carry out their tax laws. We may also disclose this information to other countries under a tax treaty, or to federal and state agencies to enforce federal non tax criminal laws, or to federal law enforcement and intelligence agencies to combat terrorism.

 

To correct your Social Security Number ("SSN"), you must fill out Part I of the Form W-9S. Include the information below with the Form W-9S to help expedite your request. After completing the form, please submit it with the information below to SOCCCD using the following methods:
